Long COVID & ME/CFS: How 24-Hour HRV Provides Clarity

A paradigm shift in medicine

Five years ago, many patients suffering from persistent fatigue, post-exertional crashes, brain fog, sleep disturbances or autonomic symptoms struggled to be understood.

Today, something remarkable is happening.

More and more physicians are recognising that Long COVID and ME/CFS are not simply problems of motivation, deconditioning or psychology. They are disorders of biological regulation—often involving profound dysfunction of the autonomic nervous system. Research increasingly supports this view, with numerous studies demonstrating altered heart rate variability (HRV) and autonomic imbalance in many patients with Long COVID.

Most published studies assess HRV using short resting ECGs (typically 5–10 minutes). While these studies already demonstrate clear autonomic abnormalities, continuous 24-hour ECG recordings provide a much richer picture of autonomic regulation by capturing sleep, recovery, physical activity, cognitive load and circadian dynamics. This makes long-term HRV particularly valuable for understanding the complex physiological fluctuations experienced by patients with Long COVID and
ME/CFS.

At Autonom Health, we have been working with autonomic regulation and 24-hour HRV for almost two decades.

What initially found application in preventive medicine, executive health and performance diagnostics is now becoming increasingly relevant for a completely different group of patients.

Over the past months, we have observed a clear trend:
More clinicians are referring patients with Long COVID and ME/CFS to us-not because HRV provides a diagnosis, but because it helps us understand how the autonomic nervous system behaves during everyday life.

For many patients, this is the first objective explanation of why apparently simple daily activities can trigger hours or even days of deterioration. A 24-hour HRV recording often makes invisible dysregulation visible. It helps identify individual limits, understand recovery capacity and establish safe physiological boundaries for pacing-one of the few internationally accepted management principles for Long COVID and ME/CFS.

What is equally encouraging is that the interest no longer comes only from individual physicians.

Self-help organisations for people living with ME/CFS are approaching us.

State-run rehabilitation facilities are seeking collaboration to integrate objective autonomic assessment into patient care.

To us, this is an important signal.

Medicine is moving from asking: “What disease does this patient have?” towards asking: “How is this patient’s regulatory system functioning?”


That shift has enormous implications-not only for Long COVID and ME/CFS, but for burnout, chronic stress, overtraining, persistent exhaustion and many other conditions where conventional diagnostics often fail to explain why patients feel as ill as they do.

Our goal has never been to replace clinical medicine. Our goal is to provide clinicians and patients with an objective window into the biology of human regulation.

Because understanding the autonomic nervous system is not the end of the diagnostic journey.

For many patients, it is finally the beginning.
